We offer school and sports physicals for children and adolescents. These visits include a review of immunization history, a physical examination, sports clearance questions, and completion of school forms. Confidential health information will be shared with the school only with parent/guardian consent. School physicals are self-pay at a flat fee of $50 per child, collected at the time of the visit. To begin the process, initial paperwork may be completed below. Appointments can be made by phone once paperwork has been completed.
Primary Care services are available at our Campus for Health & WellBeing, located at 2202 S. 11th St. in Lincoln.
