Financial Agreement

CenterPointe is a private, non-profit corporation which relies on fees from services delivered for continued operation. We want you to understand your financial responsibility for the services provided to you and your potential responsibility to make payments to CenterPointe according to the policies outlined below.


All Individuals Served

At the time of your Intake for services you will complete Informed Consent, which will include collection of financial information and completion of a financial agreement. The financial information and financial agreement are updated annually. If your address, phone number or financial circumstances change, please notify CenterPointe staff. CenterPointe will NOT deny services due to inability to pay and utilizes a sliding scale fee to determine your potential liability for all services. Loss of Insurance or funding sources may impact your financial responsibility for services at CenterPointe. Timely Payment is expected for assigned co-pays, deductibles/co-insurance amounts, or balances due (medication management and assessments, individual and group counseling services).


Medicaid, Medicare, and Private/Commercial Insurance Holders

All insurance coverage must be disclosed at the time of your intake.
CenterPointe attempts to bill Private/Commercial insurance first for all eligible claims, followed by Medicare/Medicaid, and finally to the appropriate region with the Nebraska Division of Behavioral Health (for behavioral health services, as applicable). CenterPointe verifies insurance benefits and will provide a good faith estimate of possible expenses that may be incurred for services delivered. This list is not exhaustive or comprehensive, it is an good faith estimate. Our office will file insurance claims on your behalf for the eligible services provided, but you are responsible for the fees not covered due to deductibles, co-pays, and out of network provider rates.
Changes in your insurance status with Medicaid or private insurance companies may result in your responsibility for payment in full for the services provided, in line with our sliding fee scale. You may want or benefit from services that are not covered under your insurance. If no alternate funding can be determined, you are responsible for payment in full for these non-covered services.
Only MSW, APRN, LIMHP, and LMHP credentialed provider visits are eligible for reimbursement from Medicare at CenterPointe.
Deductible and co-insurance amounts payable by you are due upon receipt of your statement, unless other arrangements have been made. Credit and debit cards can be used for one-time payments or to set up recurring payments (Visa and MasterCard only).
Accounts with balance due amounts over $200 may be subject to scheduling limitations until the account is up to date. If you would like to set up a monthly payment plan, please contact our business office at (402) 475-8717 x143. Your account may be referred to a collection agency if we are unable to resolve your balance due.

Uninsured and High Deductible Plans

Individuals may qualify for a sliding fee according to income and family size. Individuals with high deductible insurance plans may also be eligible for sliding fee rates. Some services may have a hardship exemption from paying fees. If you are not income eligible and uninsured, you have the option of paying full cost for the services provided to you. CenterPointe's Community Transitions program costs include $510 for rent and $250 for room and board (program fees).
