If requesting any services at our outpatient location, an intake is required prior to beginning services. Please complete our intake form on our website or our clinic support staff would be happy to assist you in person. All other CenterPointe services will require an intake at the time of admission.
If you need an evaluation, comprehensive assessments are available on a walk-in basis Monday through Friday; 8am to 5pm. No appointment necessary.
Psychiatric diagnostic assessments are scheduled and available within 3-5 business days of request.
To establish Primary Care Services, please call for an appointment.
