Financial Assistance

CenterPointe accepts Medicaid, Medicare, private insurance, and self-pay; however, we do not deny services to individuals based solely on inability to pay. For individuals who do not meet the requirements for state or Medicaid funding or cannot afford co-pays with private insurance, and who have an annual household income at or below 200% of Federal Poverty Levels, we provide sliding fee services at rates that are discounted from our usual rates. The discounted rates offered are determined by the recipient's household income and family size and are subject to residency in the State of Nebraska. A financial intake service must be completed to determine eligibility. Additionally, we offer flexible payment plans for any costs that are not covered by another payer.

All costs listed are subject to change without prior notice. Please be aware that this list is not exhaustive, and additional time, resources, or treatments may result in cost adjustments. The rates listed do not reflect insurance rates, allowances, and co-pays, and it is recommended that you check with your insurance agency for specific questions, exclusions, and considerations. Medications, vaccinations, and laboratory services are provided at cost, and in addition to the cost listed here—they are not eligible for discount. Please note that these rates are not applicable to Medicaid or Medicare insurance holders, and Primary Care services may not qualify for some state funding.

How to find your fee:

First, determine your household size. Household size for this purpose is determined by counting anyone that you would file taxes with, even if you don’t typically file taxes. This could include a spouse, parent, child, and/or any dependents. Next, determine your annual household income. Income for this purpose includes all earned income, child support, Disability, and Social Security income. SNAP benefits (food stamps), TANF (temporary assistance to needy families), loans, and non-recurring, irregular payments are not included in household income. If you have questions about what qualifies as income, please call our office directly. Once you have determined your household size and annual household income, find where they intersect in the first table; this identifies your Self-Pay category.
