Our clinic provides comprehensive, ongoing primary care for individuals of all ages. During these visits, we manage chronic conditions, preventive care, routine screenings, medication management, and wellness counseling. We pride our integrated approach with mental health and substance use care, to address an individual's overall wellbeing and provide the best quality care. Primary care visits are billed to your insurance according to your plan benefits, and self-pay at the time of service is available if individual does not carry insurance. To establish primary care services, individuals may walk in to the clinic or begin by completing initial paperwork below. Appointments are scheduled on a same day/next day basis and can be scheduled by calling the clinic.

- Management of chronic conditions (high blood pressure, high cholesterol, diabetes)
- Tobacco cessation counseling (for those trying to quit)
- Adult immunizations
- Allergy management
- Sick visits (cough, cold, runny nose, sore throat, earache, pink eye, urinary tract infection)
- Influenza testing
- Injuries (minor cuts, scrapes, sprains, strains)
- STD screening
- HIV and Hepatitis screening
- Care coordination between physical health and behavioral health care providers
- Electronic health records
