We provide Department of Transportation (DOT)/Federal Motor Carrier Safety Administration (FMCSA) physical exams required for commercial drivers. These exams include a comprehensive medical history, physical assessment, vision and hearing screening, and completion of DOT-required forms. Drug and alcohol testing can be performed, if applicable. DOT physicals are paid by the patient or employer at the time of service, at a rate of $150 per examination. Results are shared with the employer or regulatory agency as required by law. To begin the process, initial paperwork may be completed below. Appointments can be scheduled by calling the clinic following completion of intake paperwork.
