Primary Care

Financial Disclosure

Payment is required in full prior to being seen by a provider. These services operate on a self-pay basis for most non-established services, and do not establish an ongoing primary care relationship.  

Review the financial responsibility for your visit. 

 

Department of Transportation

DOT physical examinations are cash/self-pay only and are not billed to insurance.  

The full DOT exam fee is due prior to the exam. Additional testing required for DOT certification (if applicable) may incur additional charges.  

 

Occupational Health Visit 

Occupational health services are billed directly to the employer when employer authorization is provided. 

If employer authorization is not available at the time of the visit, the patient may be required to pay upfront and seek reimbursement from the employer. 

Services not authorized by the employer may become the patient’s financial responsibility. 

 

School Physical 

School physicals are self-pay and not billed to insurance. 

The cost is $50 per child, due prior to the appointment. 

This fee includes the physical exam only. Additional services, testing, or forms may result in additional charges. 

 

Urgent/Acute Visit 

Urgent or acute visits are self-pay for the provider visit itself. If additional labs, diagnostic tests, or procedures are ordered during your visit: 

  • Those services may be billed to your insurance, if applicable 
  • Insurance coverage is not guaranteed 
  • You are responsible for any charges not covered by insurance, including deductibles, copays, or denied claims 

You have the right to receive a Good Faith Estimate explaining how much your medical care is expected to cost if you do not have insurance or choose not to use insurance.  A Good Faith Estimate includes the expected total cost of non-emergency services, including the visit and any related services we reasonably anticipate. If you receive a bill that is $400 or more higher than your Good Faith Estimate, you may have the right to dispute the bill.  You may request a Good Faith Estimate before scheduling or at any time prior to your visit. 

 

Payment for the visit is due prior to the appointment. The Good Faith Estimate for urgent visits reflects the visit fee only and may not include costs for additional charges that are determined necessary during the visit. Additional charges may be billed separately after services are completed.
