Thank you for choosing CenterPointe for your healthcare needs. We look forward to providing you with individualized, comprehensive health care focusing on wellness and prevention. As your physical and mental wellbeing is important to us, our staff work closely in a “team approach” to support your care. Our psychiatric services provide ongoing medication and monitoring to individuals experiencing mental health symptoms, while our primary care services provide care for preventing, treating, and curing common illnesses. Together, these services promote better mental and physical health.

We have provided the following information to ensure your first appointment goes smoothly. Please complete the attached forms and bring them to your first appointment, along with your insurance identification card and photo ID.

For primary care, during your initial visit, we will be reviewing your health status and these forms contain information necessary to complete the process. You will need to arrive 15 minutes prior to the scheduled appointment so that we can make sure to have your chart ready. Before you visit, please notify your health insurance company of your new primary care provider, if required. bring or arrange to have sent any relevant medical records PRIOR to your appointment so that our provider has time to review them. This includes any immunization records for pediatric individuals.

During your initial appointment, your provider will review your health/psychiatric history. This will allow the provider to make the most informed decision on your care and work collaboratively with you to best meet your needs. Each appointment, they will evaluate the effectiveness of the treatments and make adjustments or recommendations, as needed. Services will be available as long as necessary. Please indicate to staff any supports or family you would like to be involved in your care.

While we understand that situations may occur in which you must cancel your appointment, it is requested that you notify us more than 24 hours in advance if you’re unable to make an appointment. 

In the case that you have greater than 2 no shows in a 6-month period, in our medical clinic, it would result in the inability to schedule an appointment with your provider. Instead, you will be expected to utilize the provider walk-in clinic times for a period of six months prior to returning to regularly scheduled visits. The clinic staff will notify you of the walk-in clinic timeframes. Once again, we would like to thank you for choosing us as your primary health care provider. We look forward to working with you.
